Gleason 9 and 10 prostate cancers represent, potentially, the most dangerous of the newly diagnosed, non-metastatic, forms of prostate cancer because they have the highest likelihood of distant spread. However, a significant portion of these men will still be able to achieve a cure or durable remission with the proper treatment. Even more good news is that the recently FDA-approved PSMA PET scan offers men markedly improved staging compared to what was available before, which means a patient is more likely to receive appropriate treatment upfront. Here, PCRI's Alex asks Medical Oncologist Mark Scholz, MD, all about Gleason 9 and 10 prostate cancer in 2023.
0:06 In Gleason 9 prostate cancer, what is the significance of the volume of cancer graded as Gleason 5 and the amount graded as Gleason 4?
2:41 What are the cure rates in men with Gleason 9 prostate cancer?
3:17 Is focal therapy an option for Gleason 9?
4:39 Does radical treatment make more sense for a Gleason 9 to have the best chance of achieving a cure?
5:16 If someone with localized Gleason 9 is considering focal treatment, should they also have hormone therapy since it is a high-risk cancer?
7:00 Which forms of radiation are most effective for Gleason 9?
7:56 The two types of brachytherapy are "permanent" and "temporary" seed implants. Is one any better than the other?
8:23 How common is Gleason 10?
8:58 What is the "standard" protocol for Gleason 9 and 10 prostate cancer?

Jessica was about 14 weeks pregnant when she found a lump in her breast. Going through chemotherapy before he was born and multiple surgeries after his birth led to uncertainty, fear, and despair for her and her husband Rafael. They learned that asking for help was an important step in facing these challenges. Both Jessica and Rafael joined the Breastcancer.org virtual meetup groups to connect with others affected by breast cancer. Through their participation, they found a community that offered support, healing, and a sense of belonging.

No two cancers are the same and neither should your nutrition be. When diagnosed with cancer or at risk of cancer – one immediate question is “What Should I Eat Differently Starting Now?” and “Which Supplements Should I Take and Avoid”. The answer is “It Depends”. It depends on which genetic mutations your tests identified. It depends on which cancer, which ongoing treatments and supplements. It depends on your age, weight, height, lifestyle, allergies and habits. The answer to “What Should I Eat Differently” and “Which Supplements Should I Take and Avoid” for TP53+ Chronic Lymphocytic Leukemia (CLL) also requires understanding of how the food and supplements included in your diet manipulate the molecular pathways which are relevant for cancer and interactions with your treatment.
Internet search to find answers will never be personalized to your cancer type such as TP53+ Chronic Lymphocytic Leukemia (CLL) and lifestyle conditions. Cancers are driven by biochemical pathways and therefore the answers to nutrition questions have to be based on molecular science irrespective of whether one understands it or not.
For example foods like Cabbage and Cauliflower are recommended when undergoing treatment for TP53+ Chronic Lymphocytic Leukemia (CLL).Similarly Neem Extract supplements are recommended with treatment for TP53+ Chronic Lymphocytic Leukemia (CLL). So always personalize your nutrition to your cancer gene mutations and other factors.

Brian has been my rock throughout this entire cancer journey that we've been so unexpectedly thrown into. Honestly, if this were the other way around, I'd be a complete and absolute mess. He has been by my side EVERY single step of the way. He gives me my strength when I am at my lowest.
In some ways, I think a cancer diagnosis can be even harder on those we love than the person who is diagnosed. There are so many "what-ifs" we have to ponder on a daily basis, all while being strong and not giving up the fight.
For today's video, I wanted him to share with you how he is doing because I think it's such an important thing to talk about. From the moment I was diagnosed with stage III ovarian cancer, his entire world has been turned upside down as well. I hope by him sharing, maybe it can help others who are helping their loved ones through this process.
PEEF, Hope & Love,
Jessica & Brian ❤️
