Early-stage stomach cancer can be a silent threat, often causing few or no symptoms until the disease has grown into advanced stages. It is the time when cancer has not spread to other parts of the body and if detected early is curable.

Staging is the process of finding out how much cancer is in a person's body and where it's located. It's how the doctor determines the stage of a person's cancer. The factors that decide the stage of stomach cancer are as a percentage of area affected by the walls of the stomach, damage done to the lymph nodes & area affected other than the lymph nodes.

The inner lining of the stomach is called the mucosa. The lamina propria is the thin layer that consists of connective tissues. The submucosa is the second layer that supports the mucosa. It is rich in blood vessels, lymphatic vessels, and nerves. The serosa is the last and outermost layer. It’s the lining that wraps around the stomach to confine it.

The early-stage stomach cancer is only involved in mucosa, submucosa & muscle layer. An expert advises a patient to undergo endoscopy as well as endo sonography to be sure. If the stomach cancer is in the mucosa, it can be treated by endoscopy itself.
Treating stomach cancer with endoscopy must be done by a specialist and an experienced doctor only.

Adenocarcinoma of the stomach is a common cancer of the digestive tract worldwide, although it is uncommon in the United States. It occurs most often in men over age 40. This form of gastric cancer is very common in Japan, Chile, and Iceland.

The rate of most types of gastric adenocarcinoma in the United States has gone down over the years. Experts think the decrease may be because people are eating less salted, cured, and smoked foods.

You are more likely to get gastric cancer if you:

Have a family history of gastric cancer

Have an infection of the stomach by bacteria called Helicobacter pylori

Had a polyp larger than 2 centimeters in your stomach

Have inflammation and swelling of the stomach for a long time (chronic atrophic gastritis)

Etiology

Helicobacter pylori infection is the cause of most stomach cancer. Autoimmune atrophic gastritis and various genetic factors: Gastrointestinal Stromal Tumors are also risk factors. Dietary factors are not proven causes.

Gastric polyps can be precursors of cancer. Inflammatory polyps may develop in patients taking NSAIDs, and fundic foveolar polyps are common among patients taking proton pump inhibitors. Adenomatous polyps, particularly multiple ones, although rare, are the most likely to develop cancer. Cancer is particularly likely if an adenomatous polyp is 2 cm in diameter or has a villous histology. Because malignant transformation cannot be detected by inspection, all polyps seen at endoscopy should be removed. The incidence of stomach cancer is generally decreased in patients with duodenal ulcer.
Pathophysiology
Gastric adenocarcinomas can be classified by gross appearance:

Protruding: The tumor is polypoid or fungating.
Penetrating: The tumor is ulcerated.
Superficial spreading: The tumor spreads along the mucosa or infiltrates superficially within the wall of the stomach.
Linitis plastica: The tumor infiltrates the stomach wall with an associated fibrous reaction that causes a rigid leather bottle stomach.
Miscellaneous: The tumor shows characteristics of 2 of the other types; this classification is the largest.
Prognosis is better with protruding tumors than with spreading tumors because protruding tumors become symptomatic earlier.
