El doctor Jorge Adeva Alfonso vídeobloguea desde el laboratorio oncológico del Hospital 12 de Octubre acerca de los dos tipos cáncer de esófago más prevalentes en la sociedad, los tumores que prosperan después de la garganta, gracias al hábito tabáquico o al exceso de alcohol, y las neoplasias ocasionadas por el reflujo gástrico en la zona más distal del esófago o boca de entrada al estómago. <br /> <br />Mientras los primeros, que se denominan escamosos, han tendido a disminuir su incidencia en la población durante los últimos años, los segundos, que forman el llamado esófago de Barrett, se han convertido en "casi una epidemia" en las dos últimas décadas. <br /> <br />Las primeras señales que indican que podemos padecer un carcinoma esofágico aparecen cuando tenemos dificultades para tragar, estamos experimentando dolor torácico, perdemos peso o no se nos abre el apetito; síntomas por los que debemos acudir sin pausa al médico de cabecera o al especialista de aparato digestivo. <br /> <br />Para diagnosticar un cáncer de esófago se recurre a la gastroscopia: una vez sedado el paciente, se introduce una sonda por su nariz hasta que se llega a la zona de exploración, donde se efectuará una biopsia. La muestra extraída se enviará al anatomopatólogo. <br /> <br />En ocasiones, el endoscopista puede hacer también una ecoendoscopia aprovechando la gastroscopia. El galeno observa la zona tumoral con el ecógrafo y estudia el carcinoma in situ. <br /> <br />"Evaluado ya el cáncer de esófago, si está muy localizado y delimitado, se puede hacer un tratamiento curativo a través de la propia endoscopia, resecando el tumor sin necesidad de operar. Esto solo se debe hacer en centros de alta experiencia como el Hospital 12 de Octubre", informa el doctor Jorge Adeva. <br /> <br />Cuando el desarrollo del tumor todavía está en una fase primaria, se puede individualizar al paciente: "O bien se lleva a cabo una cirugía mayor o bien se le trata con radioterapia o quimioterapia para evitar la cirugía", explica. <br /> <br />Si el cáncer se ha diseminado y se ha pasado a otros órganos del cuerpo humano, metátasis, se ofrece un tratamiento paliativo que mejore su calidad de vida y expectativa de supervivencia. <br /> <br />"Desde hace un quinquenio, además de las terapias clásicas de radioterapia y quimioterapia, se administra la proteína HER2 a un grupo determinado de pacientes con cáncer de esófago", detalla Adeva. <br /> <br />La proteína HER2 es clave en el crecimiento y división de las células, por lo que se ha convertido en una diana de los tratamientos oncogénicos. <br /> <br />"Como ya sucediera en grupos de pacientes con cáncer de mama, la efectividad es mayor si a la proteína se añaden el tratamiento con quimioterapia clásica y un fármaco que se llama trastuzumab", asegura. <br /> <br />Y todo gracias a los voluntarios que participan en los ensayos clínicos para encontrar y probar nuevas terapias. <br /> <br />"En este caso, la suerte se alió con la investigación. En el laboratorio del Hospital 12 de Octubre buscamos nuevas dianas para atacar los procesos cancerígenos -refrenda.
