Familial Pancreatic Cancer

Clinical Research Coordinator and Associate Genetic Counsellor Tanya Dwarte presents a webinar called Genetic Testing and Inherited Pancreatic Cancer Risk.
The webinar highlights the current understanding of sporadic and inherited forms of pancreatic cancer and provide an overview of inherited cancer syndromes and the associated pancreatic cancer risks. Information about the process of a genetics assessment, including consideration of genetic testing for pancreatic cancer patients or for those with a family history of the disease, will be addressed.

What causes Pancreas Cancer? What are the examinations needed to diagnose Pancreatic Cancer? The pancreas is important in blood sugar regulation because it produces insulin and glucagon, two chemicals that regulate blood sugar levels. Pancreatic cancer is cancer that's found anywhere in the pancreas. Pancreatic cancer is seldom detected at its early stages when it's most curable. This is because it often doesn't cause symptoms until after it has spread to other organs.

01:32 - Presenter Introductions
02:20 - Pancreatic Cancer: The Problem and Risk Factors
05:06 - Localize Disease and Defining Respectability
15:13 - Metastatic Disease

Question and Answer Session:
55:22 - When should you start getting tested if you have cancer in your immediate family history?
59:10 - On the slide that shows Treatment Sequencing, is the 2nd line only effective if the cancer is genetic? If not, what are the things my doctor should be looking for to determine which of these might be best?
1:01:58 - You mentioned that a chaplain was at your multidisciplinary meetings, do you have palliative care involved as well?
1:04:09 - I was prescribed glutamine powder and alpha-lipoic acid for mild neuropathy for my fingers. Is it okay to take with folfirinox?
1:06:15 - Following a diagnosis, is there any opportunity to actually remove the entire pancreas & replicate the pancreas' function?
1:07:33 - I have Stage 1B locally advanced; did 3 cycles of folforinox & then changed to Gem-Abraxane N59 size of tumor for first time, but still abutted to SMA and SMV. How much of a fat plane needs to be had before moving to surgery (whipple)? Is it possible tumor will be mostly scar tissue that is shown in CT after 6 months of chemo?
1:09:59 - Is a pharmacist on the team?
1:10:22 - Spouse has an ATM mutation. After a recurrence last fall, he tried Gem/Abraxane then Folfirinox, but the cancer keeps growing. His Oncologist is moving him to SBRT, then Opdivo. Is this a therapy you have seen, after chemo fails to suppress the cancer?
1:12:50 - Is there any relationship with development of pancreatic cancer & Helicobacter pylori?
1:13:38 - Spouse has breast, pancreatic & stomach cancer family history. No prior family history genetic testing. Should she obtain genetic counseling?
1:15:09 - This is a question for a patient that is 3.5 years out from diagnosis of pancreatic adenocarcinoma. Treatment consisted of a Whipple procedure followed by 6 months of gemcitabine. The patient has been NED since treatment; however, they have experienced intermittent fever/chills since, sometimes 2 or 3 times per month. Are there any available treatments you could recommend for this?
1:17:17 - Do treatment outcomes differ radically by age. Do we have much data on if a 30 year old (BRCA 2) has different outcomes than someone who is 50? Are there different treatment protocols for age?
1:21:56 - If a patient with a BRCA2 mutation has been NED for 5 years, should they continue to receive follow up and, if so, how long?
1:24:23 - Should a patient undergo genomic testing/molecular profiling of the tumor after undergoing extensive treatment, even if that testing was done at the time of diagnosis?
The Patient Webinars: Pancreatic Cancer occurred in June 2021. This informational program was created in conjunction with the recently updated NCCN Guidelines for Patients® for Pancreatic Cancer.
The NCCN Guidelines for Patients are available to read and download for free online and via the NCCN Patient Guides for Cancer mobile app. Printed editions can be ordered from Amazon.com for a small fee.
