When Donald was 11-years-old, he was treated for a bladder infection. The instrument used to look in his bladder accidentally injured a small section of his urethra, the tube that carries urine out of the body.
The injured area develop scare tissue also known as a urethral stricture, which narrowed the urine tube, restricting the flow of urine and causing bleeding and pain every time Donald urinated.
For sixteen years, Donald lived with the pain. As his condition worsened, he would have procedures to relieve the scare tissue and dilate or expand the urine channel, but the treatments were temporary. Over time, the injured spot of the urethra would narrow and his symptoms would return.
Eventually, the procedures were needed so frequently that Donald sought the expertise of Dr. Christopher Cantrill, a urologist with Urology San Antonio who specializes in reconstructive surgical repairs to the urinary system.
Dr. Cantrill performed a urethroplasty to fix the damaged section of the urethra. In what is considered a substantial operation, Dr. Cantrill removed the 3 centimeters of damaged urethra and stitched the two ends of the urethra together, forming a clear, water-tight passageway for urine to flow from the body.
The operation was a success. Donald no longer faces pain with urination. He says the restoration of normal function has changed his life.

Familial adenomatous polyposis: A genetic disease characterized by the presence of numerous precancerous polyps in the colon and rectum. The polyps usually begin to form at puberty, and colon cancer almost always develops later in life. Abbreviated FAP. FAP is inherited as an autosomal dominant trait. Most people who receive the gene manifest the disease, although the expression of FAP can vary markedly from person to person. The gene that is mutated in FAP is the APC (adenomatous polyposis coli) gene on chromosome 5. Surgery is often necessary to remove the colon in order to prevent the development of cancer. A milder type of familial adenomatous polyposis has been identified that is inherited in an autosomal recessive manner. Srdan Verstovsek, MD, PhD, professor in the Department of Leukemia and director of the Hanns A. Pielenz Clinical Research Center for Myeloproliferative Neoplasms at The University of Texas MD Anderson Cancer Center, discusses how to control symptoms while using hypomethylating agents in patients with myelofibrosis (MF).
The hypomethylating agents are used solely to lower blast and control disease. In some cases, complete responses and partial responses with these agents have been reported, Verstovsek says, but the goal of these agents is to put the disease into a chronic phase with less than 10% blast so that the patient can receive transplant.
According to Verstovsek, the quality of life (QoL) on these agents is not great while MF in general has many systemic symptoms that cannot be controlled by hypomethylating agents. However, to control the symptoms, JAK inhibitor ruxolitinib (Jakafi) can be given in combination to help with QoL and the progressive nature of the disease.
Ruxolitinib, the only approved JAK inhibitor for this setting, is endorsed by the NCCN guidelines for these patients, regardless of their blood cell count, to control symptoms of MF, Verstovsek says.
