Gallbladder cancer is an abnormal growth of cells that begins in the gallbladder.

Your gallbladder is a small, pear-shaped organ on the right side of your abdomen, just beneath your liver. The gallbladder stores bile, a digestive fluid produced by your liver.

Gallbladder cancer is uncommon. When gallbladder cancer is discovered at its earliest stages, the chance for a cure is very good. But most gallbladder cancers are discovered at a late stage, when the prognosis is often very poor.

Gallbladder cancer may not be discovered until it's advanced because it often causes no specific signs or symptoms. Also, the relatively hidden nature of the gallbladder makes it easier for gallbladder cancer to grow without being detected.

Symptoms: ---
Gallbladder cancer signs and symptoms may include:

1. Abdominal pain, particularly in the upper right portion of the abdomen
2. Abdominal bloating
3. Losing weight without trying
4. Yellowing of the skin and whites of the eyes (jaundice)


Risk factors:----
Factors that can increase the risk of gallbladder cancer include:

Your sex. Gallbladder cancer is more common in women.
Your age. Your risk of gallbladder cancer increases as you age.
A history of gallstones. Gallbladder cancer is most common in people who have gallstones or have had gallstones in the past. Larger gallstones may carry a larger risk. Still, gallstones are very common and even in people with this condition, gallbladder cancer is very rare.
Other gallbladder diseases and conditions. Other gallbladder conditions that can increase the risk of gallbladder cancer include polyps, chronic inflammation and infection.
Inflammation of the bile ducts. Primary sclerosing cholangitis, which causes inflammation of the ducts that drain bile from the gallbladder and liver, increases the risk of gallbladder cancer.

पित्ताशय की थैली में हुई कैंसर कोशिकाओं की असामान्य वृधि को पित्त का कैंसर या पित्ताशय का कैंसर कहा जाता है। पित्त का कैंसर काफी दुर्लभ है इसलिए इसकी पहचान कर पाना काफी मुश्किल होता है। पित्त की थैली एक नाशपाती के सामान दिखाई देती है और यह पेट पेट की दाईं ओर लिवर के नीचे स्थित होती है। पित्त की थैली, लीवर द्वारा बनाए गये पाचन तरल उत्पाद को संग्रहित करती है।

जब पित्ताशय की थैली के कैंसर का पता उसके शुरुआती चरणों में लगाया जाता है, तो इलाज की संभावना बहुत अच्छी होती है। लेकिन अधिकांश पित्ताशय की थैली के कैंसर का पता देर से चलता है, जब तक कैंसर की पहचान की जाती है तब तक स्थिति काफी खराब हो चुकी होती है।

पित्ताशय की थैली के कैंसर की खोज तब तक नहीं की जा सकती जब तक कि यह उन्नत न हो जाए क्योंकि यह अक्सर कोई विशिष्ट लक्षण या लक्षण नहीं पैदा करता है। इसके अलावा, पित्ताशय की थैली की अपेक्षाकृत छिपी प्रकृति पित्ताशय की थैली के कैंसर का पता लगाए बिना बढ़ना आसान बनाती है।

In this video, Dr. Praveen Kammar (Consultant Surgical Oncologist, SSO Hospital, Mumbai) talks to us about whether gallstones cause gallbladder cancer.

Gallstones are a risk factor for gallbladder cancer. Out of 5 gallbladder cancer patients, 4 have gallstones problem. This tells us that gallbladder cancer has a close relation to gallstones.

Only 1% of the patients with gallstones are detected with gallbladder cancer. But why do gallstones cause gallbladder cancer?

Scientifically it is found that when the gallstone grows bigger than 3 cm in the body it creates a risk of gallbladder cancer. Therefore, it is advised that people should identify the early symptoms of gallbladder stones at the beginning itself.

If the gallstone is smaller than 2 cm and does not show any complications, it is not necessary to treat such gallstones surgically.
If the gallbladder stones are to be treated surgically then doctors use methods such as keyhole surgery or laparoscopic surgery which have proved to give excellent results.

Incidental gall bladder cancer management is challenging and needs a multi-disciplinary approach. This talk summarises the latest surgical management strategies for incidental gall bladder cancer. As we see in the presentation, incidental gall bladder cancer has to be differentiated from missed gall bladder cancer. The aim is to spread awareness on this topic and identify the "high-risk gall bladders" so as to avoid missing gall bladder cancer. The management relies on few key factors such as bile spillage in laparoscopic cholecystectomy, port site management, use of endobags for specimen retrieval, stage of the disease on final histopathology, and so on..

All these factors when considered in the management of these patients give a better outcome. We prefer giving neoadjuvant therapy to patients with bile spillage as that helps us tests the biology of the disease. Radical Cholecystectomy is the treatment of choice for cases beyond T1a.
