The patient in this video is a 63 year old lady, a housewife. She was presented with a black mass on lateral aspect of the right lower eyelid; the mass was growing slowly for 5 years. She is from Guntur, India and is treated by Dr. Debraj Shome, oculoplastic and facial plastic surgeon and director of The Esthetic Clinics (http://www.theestheticclinic.com) and a consultant of Saifee Hospital and Apollo Spectra Hospital in Mumbai India. She is trained in USA, Singapore, Europe and India.
It is said a high index of suspicion of cancer must be maintained in any mass on the eyelid and face. The masses may appear small but cancer spreads beneath the normal skin.
An image in the video shows how much tissue will be removed when it is tested for cancer. A superior eyelid flap was taken to close the lower eyelid defect. A periosteal flap is raised and the posterior lamella of the eyelid created. An image in the video shows this.
6 weeks post operation, the eyes looks much symmetrical and the mass is removed. The patient according to the Doctor has been cancer free over 10 years of follow-up.

The incidence of skin cancer is greater on the lower eyelid and inner canthus of the eye, but the upper eyelid may also be compromised. The most common malignant tumor of the eyelid skin is basal cell carcinoma, followed by squamous cell.
The former is by far the most frequent type. Basal cell typically grows at a slow rate and has a relatively benign behavior. Squamous cell carcinoma, on the other hand, is very aggressive and tends to spread to other organs.
Treatment for eyelid skin cancer is always surgical. If the mass is detected early on the course of the disease, the operation to remove the tumor can be quite simple and may only require a small reconstruction.
On the contrary, advanced carcinomas demand the removal of much tissue, and consequently, require very complex reconstructive surgery.

Ever been told you had an Eye Freckle? This freckle or “choroidal nevus” is concerning as it could become an ocular melanoma, a potentially deadly form of eye cancer. Learn what testing can be done, the diagnosis and what treatments are available.
There are many different pigmented spots that can appear in the eyeball and one of these spots appears as a freckle or mole behind the retina. This retinal nevus or “choroidal nevus” is basically a mole behind the retina of the eye and could potentially become a melanoma. Melanoma of the eye is a type of eye cancer that can seriously threaten a person's eyesight as well as their life.
These choroidal nevi are typically found in about 7-10% of the population but thankfully most people never develop a choroidal melanoma. However, due to the severity of the diagnosis, eye doctors take the evaluation of choroidal nevi very seriously.

See my latest CME article on eyelid reconstruction:
http://www.slideshare.net/wtmc....clellan/eyelid-recon
This is a surgical video demonstrating a complex lower eyelid reconstruction following skin cancer resection. The skin cancer has been removed by a specialized dermatologist called a moh's surgeon. Once finished with the moh's portion of the procedure the patient is then placed under anesthesia for the eyelid reconstruction. The eyelid has multiple layers which need to be repaired in an ordered fashion. In addition there are post operative scar forces which must be considered before and during the surgery. The periosteal bone flap as described here was designed and reported by Dr. McClellan at the American Society of Plastic Surgeons meeting in 2010 in Toronto. This is now a workhorse for lateral eyelid posterior lamella reconstruction. If you are having eyelid reconstruction please seek out a Board Certified Plastic Surgeon for repair.
