Since 1997, UCLA's Comprehensive Mesothelioma Program has been using lung-sparing surgery and innovative postoperative therapies that stress quality of life as well as survival. Patients benefit from UCLA's expertise in performing a meticulous procedure (pleurectomy and decortication) that removes all visible tumor from the chest while sparing normal structures, such as the lung, pericardium and diaphragm.
The surgery is followed by precise radiation therapy to target remaining cancer cells and avoid injury to the preserved lung. Patients also benefit from a number of existing and investigational therapies to target the remaining microscopic disease and slow its re-growth.
Recognizing that mesothelioma cannot be entirely eliminated from the body, our physicians treat it as they would any chronic condition. That means we use frequent (often daily) well-tolerated therapies to slow its growth rather than using concentrated treatments whose toxicities may be excessive. UCLA has pioneered the use of therapies such as immunotherapy and cryoablation, which have led to significant advances in the care of patients with this condition. Dr. Robert Cameron founded the program.
Treatment of sarcomatoid mesothelioma.
Patients with sarcomatoid mesothelioma benefit from UCLA's expertise in this rare disease. We have found that chemotherapy agents typically used to treat sarcomas in other areas of the body are more effective in these patients than are the typical mesothelioma chemotherapy agents. This chemotherapy, followed by surgery and radiation, has provided significant advantages for patients in this small group. All patients are treated by a multidisciplinary team. A dedicated nurse practitioner helps to coordinate care and is available to answer questions from patients and referring physicians.

Our Services.
Patients with all stages of mesothelioma are provided with expert surgical, radiation and medical oncology services.
Surgical techniques include lung-sparing tumor surgery (pleurectomy and decortication or P/D), lasers, bronchial (airway) and esophageal stents and video-and computer-assisted robotic surgery.
Radiation therapy techniques include stereotactic body radiation therapy (SBRT), intensity-modulated radiation therapy (IMRT) and radiosurgery using the Novalis Tx image-guided system.
Chemotherapy, immunotherapy and targeted biologic agents are chosen based on extensive experience treating mesothelioma.
What is mesothelioma?
Mesothelioma is a tumor of the tissue that lines your lungs, called the pleura. Mesothelioma usually starts in the pleura of the lungs, but it can also start in the abdomen or other organs. Often, mesothelioma presents with fluid surrounding the lungs.

Diagnosis and Screenings.
At times, it is hard to tell the difference between malignant mesothelioma and metastases to the pleura. Imaging tests and biopsies are used to make the diagnosis.
Treatments.
Traditional treatment may include surgery, radiation,and chemotherapy at UCLA, we have established a working group to manage recurrent mesothelioma with minimal invasive image guided cryoablation. Using this technique, up to 90% of local recurrent tumors can be controlled with limited complications.

CHRYSOTILE MESOTHELIOMA RISK.
The risk of mesothelioma from exposure to chrysotile asbestos.
Purpose of review.
This review assesses the risk of developing diffuse malignant mesothelioma of the pleura from exposures to chrysotile fibers and contrasts it with the known risk of amphibole asbestos.
Recent findings.
Although a rare cancer, the mortality rates of pleural mesothelioma continue to be significantly elevated because of past occupational exposures to airborne asbestos fibers. New analyses of occupational epidemiologic studies for highly exposed workers show a substantially lower potency and suggest an empiric threshold for chrysotile compared with amphibole asbestos. Important kinetic and pathological differences between chrysotile and amphiboles have been substantiated that support chrysotile's impotency in causing pleural mesothelioma.
Summary.
Excess risk of pleural mesothelioma from past exposures to asbestos, as evidenced by a trend of high incidence rates during the last half century, appears to be the result of nonchrysotile asbestiform fibers. Although scientific efforts and legal arguments continue, the risk of pleural mesothelioma in human populations is probably negligible for exposures to airborne chrysotile asbestos that is not known to be contaminated by amphibole. This distinction for asbestos fiber types is pivotal for understanding hazards and characterizing risks of continued use of natural chrysotile asbestos today and also new nanofibers.

Mesothelioma After Pneumonectomy

Objective.
We review our 24-year experience with extrapleural pneumonectomy (EPP) in the treatment of epithelioid malignant pleural mesothelioma (MPM).
Background.
Recent publications, particularly the MARS (Mesothelioma and Radical Surgery) feasibility study by Treasure et al, have questioned the safety and efficacy of EPP for MPM.
Methods.
An institutional review board–approved, prospective, single-center database was retrospectively reviewed. Descriptive statistics and Kaplan-Meier analysis of overall survival are reported.
Results.
From 1988 to 2011, a total of 529 patients with epithelioid MPM underwent complete resection by EPP as part of a multimodality strategy. Among these, 131 (25%) were women, and the median age was 59 (range, 17–79) years. Median postoperative hospital stay was 10 (range, 1–101) days. Twenty-six patients (5%) experienced 30-day or in-hospital mortality. Median overall survival was 18 months, with 1-, 3-, 5-, and 10-year survival rates of 67%, 28%, 14%, and 4%, respectively. Outcome by pathologic lymph node status (N, median overall survival) was N0: 224, 26 months; N1: 118, 17 months; N2: 181, 13 months; N3: 5, 7 months; Nx: 1, not evaluable.
Conclusions.
EPP has evolved as an effective method for macroscopic complete resection. This study confirms that lymph node status is significantly correlated with overall survival in patients with epithelioid MPM undergoing EPP and suggests that those with simultaneous involvement of N1 and N2 stations are at increased risk. This observation underscores the need for thorough staging of both N1 and N2 stations and has implications for revision of MPM staging criteria.
