This is a unique case of an optic nerve sheath meningioma type 3A as it extends beyond the optic canal into the cisternal space. Patient presented with progressive and severe visual loss and was told there are no surgical options other than transection of the optic nerve. Here we show the elegance and effectiveness of the endoscopic endonasal approach to perform an excellent decompression of the optic nerve with near-complete resection of the cisternal and intracanal portions of the tumor. Patient had an excellent outcome with no complications and reported improvement of her right-side vision.
