Management of Parathyroid Carcinoma

Parathyroid carcinoma is a very rare diagnosis. Usually, it's a suspicion.
The operation needed is parathyroid resection en-bloc with thyroid lobectomy.
The frozen section won't help you make the diagnosis as the diagnosis is made on the evidence of invasion. You can always send a frozen to sure the tissue you have removed is the parathyroid gland.
If you are suspicious of parathyroid carcinoma in the OR but doesn't see any signs of invasion still recommended to do an en-bloc resection and wait for the final pathology results
