Cervical Cancer: Brachytherapy Treatment

Dr. Brian Lawenda discussed the treatment of CERVICAL CANCER with the use of "brachytherapy." Most patients with larger cervical cancers will be treated with a combination of chemotherapy, external beam radiation therapy and internal radiation, called brachytherapy. Brachytherapy is the placement of a radioactive pellet (we use Iridium-192) directly into the cervical cancer tumor to deliver a very precise, high-dose of radiation exactly where we want it, simultaneously limiting the amount of dose to surrounding normal tissues (such as the bladder, rectum and intestines).

This treatment can be done in the operating room or in the clinic. We often will have the gynecologist place a "Smit sleeve" into the cervical canal before these treatments. The sleep is sown in place, and will removed after the final radiation treatment. We use the sleeve to make the placement of the brachytherapy device (we use a "tandem and ring brachytherapy applicator") more comfortable for the patient, as the radiation oncologist places the tandem component of the applicator through the sleeve and into the uterus at each insertion. There are usually 4-5 brachytherapy treatments, which are done twice-per-week, starting half-way through the course of external beam radiation therapy.
