Kaposi sarcoma (KS) is a tumor induced by infection with the oncogenic virus human herpesvirus-8/KS herpesvirus (HHV-8/KSHV). The endothelial cell culture model described here is uniquely suited for studying the mechanisms by which KSHV transforms host cells.

Muitas pessoas com diagnóstico de Sarcoma têm a dúvida de como surgiu este tumor?
Existem alguns fatores de risco relacionados aos sarcomas que serão ditos no vídeo, mas a realidade é que em muitos dos casos não sabemos a causa do surgimento dos Sarcomas.
Alguns pacientes poderão ter alterações genéticas que favorecem o seu aparecimento e poderão transmitir o risco para seus descendentes, mas na maioria dos casos isto não acontece.
Os principais fatores de risco para o aparecimento dos sarcomas são:
- Radioterapia prévia.
- Síndromes genéticas.
- Linfedemas.
- HIV/ AIDS

Kaposi sarcoma ( Part 6 ) : Diagnosis and staging of kaposi sarcoma : Pathology USMLE Step 1
Kaposi sarcoma
Definition:
• malignant, multifocal, highly vascularized tumor caused by the human herpesvirus 8 (HHV8 )
Classification:
• HIV-associated Kaposi sarcoma
Clinical features
• Multiple cutaneous or visceral, elevated tumors with rapid growth
• Initial manifestation on skin and mucosae (especially face/oral cavity and chest)
• Over time, organ involvement is possible.
• If the gastrointestinal tract is involved: abdominal pain and diarrhea, possibly hemorrhage and bowel obstruction if severe
Diagnostics
• Skin biopsy: spindle-shaped cells, leukocyte infiltration, and angiogenesis.
• Chest x-ray, sonography of the abdomen and lymph nodes
Treatment
• Treatment of the underlying disease → antiretroviral treatment in HIV patients
• Symptomatic therapy:
• Local: intralesional administration of vinblastine or radiation therapy
• Systemic: pegylated liposomal doxorubicin or liposomal daunorubicin
• Prognosis: very variable

Kaposi Sarcoma, nodular (tumoral) stage
This lesion is seen less nowadays since the widespread use of antivirals in the treatment of HIV/AIDS. In the US Kaposi Sarcoma is considered an AIDS-defining lesion (associated with AIDS) and is due to the virus HHV8 (humans herpesvirus 8).
The lesions present as red to purple, asymptomatic blotch/bruise. They can involve the skin as well as mucous membranes (mouth) and internal organs in later stages.
The lesions go through a few phases: patch (flat area of discoloration greater than 1 cm), plaque (thicker and lifted lesion greater than 1 cm), and nodular as in this case. Patch and plaque phases look quite similar microscopically. Nodular phase is distinct.
Microscopically (nodular stage):
1. The biggest clue is hemorrhage (RBC outside of blood vessels) within a vascular lesion. Most hemangiomas do not have red cell extravasation.
2. The lesion is within the dermis and is well circumscribed pink spindled cells with extravasated red cells. Spindled cells often run parallel to each other.
3. May see irregular slit-like vascular spaces within the main part of the lesion.
