Legal restrictions on cannabis products have been loosened in recent years, attracting interest for their possibilities in relieving side effects from breast cancer treatment. Medical marijuana is the most well-known and controversial of these products, but cannabidiol, also known as CBD, has had the more dramatic rise in availability. CBD can be found legally nationwide in oils, tinctures, lotions, and even things like shampoos and lip balms, and claims to its health benefits have been as varied as the products themselves.
Living Beyond Breast Cancer CEO Jean Sachs, MSS, MLSP, spoke to Alex Capano, DNP, CRNP, FNP-BC, the chief science officer at Ecofibre Limited, a company that makes hemp-derived products such as CBD oil. Dr. Capano shares the basics about CBD: what it is, how it differs from medical marijuana, and why people use it. She also shares tips for wading through the many products marketed as using hemp to help you find something that will be helpful to you.

Breast cancer is one of the leading causes of death among US women. It is a common form of cancer that is not particularly dangerous if detected before metastasis (spreading) to other organ systems. Some unique characteristics of breast cancer, including the presence of an activated embryonic growth gene in many breast cancer types, allows CBD to be effective not only in a palliative way, but in attenuating tumor proliferation via inhibition of this gene, ID-1, that causes the tumor to grow at the rate of an unborn fetus.

Cannabis and kidney disease have been a hot topic for most kidney disease patients. In most cases, Cannabis usually does not affect kidney function in healthy individuals. Many people use cannabis to treat a variety of problems. Most kidney patients experience chronic symptoms such as pain, nausea, anorexia, sleep disturbance, anxiety, and depression.
And most of these are approved indications for medical cannabis across the United States in many states. Anxiety, depression, and insomnia are the most common psychiatric conditions that people usually self-treat with cannabis. Most of the evidence focuses on chronic pain, nausea, anorexia, or loss of appetite. Additionally, many patients living with kidney disease and those on dialysis experience substantial chronic pain. So cannabis could have a potential therapeutic role in pain management among kidney disease patients.
Patients with baseline chronic kidney disease who used marijuana experienced more rapid eGFR decline, according to recent studies. Most studies included smoked recreational cannabis, either in healthy people, or they've evaluated any lifetime use among patients with kidney disease. There are some differences between recreational cannabis and medicinal cannabis such as the route of administration, the dose, the frequency, and all of these can have different effects on long-term outcomes.
These studies do not demonstrate any association between cannabis use and the development or progression of kidney disease. Thus, in order to answer this question, we conducted a study among people at high risk for kidney disease, and we did demonstrate an association between chronic cannabis use and a more rapid decline in estimated glomerular filtration rate (eGFR) with people with kidney disease. The use of cannabis in the United States and most parts of the world have increased significantly over the last decade, but particularly among people over the age of 50. Thus, in other words, there are studies out there that state that cannabis could have some benefits for your kidneys but you should always consult with your doctor.

So everyone knows THC, tetrahydrocannabinol. It was first discovered in 1964 in Raphael Mechoulam's lab in Israel. And it was about the same time the Beetles were first being exposed to cannabis themselves and it was the 60s and so voila. This is the chemical that's causing all the effects of marijuana is what people thought.
Before that time people had not been able, scientists had not been able to find this one abundant compound. They couldn't isolate it. They couldn't dictate the chemical structure of it. They found other compounds called cannabinoids in cannabis. But THC was the big find and a 32-year-old scientist discovered it and, you know, made his, launched an illustrious career.
But it turns out that THC is not the only game in town in marijuana or in cannabis, and is not the only extremely useful compound in cannabis. And for that you have to look at CBD. CBD stands for cannabidiol, CBD. It's basically a structural isomer of THC. What that means to say is THC and CBD have the same chemical composition but the atoms are just arranged just a little bit differently. In fact, there's a ring that opens and closes. If that ring is closed it's THC, if that ring is open it's CBD. And you can actually in a flask convert between one and the other with just a little bit of acid. And, you know, any organic chemist out there will know what I'm talking about.
So that CBD compound we've actually known about for longer than THC but it's only in the last few decades that we've begun to appreciate how useful it is both in helping to modulate the effects of THC and as a unique compound itself. It has antiseizure properties. It has pain relieving properties. It has anticancer properties. It blunts the worst effects of THC itself when it's coadministered. So people talk about dissociative effect with THC. People talk about a anxiogenic effect. When you put CBD in there it actually helps to mitigate those effects. And that's been shown in studies where they go out in the field and they test what types of marijuana people have and the ones that have CBD in them people report improved, less of those negative symptoms I've described. And that's also been seen in animal studies and on and on.
Some groups have even called for just adding CBD into cancer chemotherapy regimens and these are cancer research groups. CBD has been also hailed as a treatment to prevent diabetic neuropathy which is the leading cause of blindness in the United States. It protects nerves. And this is all from a little compound, set of compounds that this cannabis plant naturally produces. Some strains have higher amounts of THC and low amounts of CBD. Some strains have low amounts THC and high amounts of CBD and some are in between somewhere. And that's all part of the genetic diversity and chemical diversity of cannabis.
