Abstract
Introduction - Background: Basal cell carcinomas are the most common malignant tumors of the skin. The main risk factor is sun exposure on light skin types, but also chemicals and genetic factors seem to be influencing factors. There are several known subtypes, and clinical evolution and complications highly depend on the exact subtype of basal cell carcinoma.
Methods: We report the case of a 61-year-old man, with a known history of cardiovascular disease and multiple recurrences of pseudoadenoid basal cell carcinoma. He was admitted with a tumor on the medial canthus and skin of the upper and lower eyelid on the left eye. The management of medial canthus and eyelid basal cell carcinoma is complicated. The complete removal of the tumor might be challenging and it requires complex surgical techniques in order to reconstruct the area.The tumor was revealed to be a local recurrence of previously excised basal cell carcinoma, affecting the same area. The patient underwent surgical excision of the tumor within the limits of oncological safety. Furthermore, the tumor invaded the nasal bone and caused osteolysis therefore curettage was used to remove the affected area.
Results: In order to reconstruct the defect in the medial canthal area and lateral part of the upper nose, a rotated glabellar flap was used. The flap was viable and capillary pulse was normal. The surgical outcome was satisfactory and the patient was discharged with highly recommended surveillance.
Discussion - Conclusion: Unfortunately, recurrence rates for basal cell carcinomas seem to be high and short period follow-up is mandatory. The patient is at high risk, since he presented with several recurrences already, whereas the last recurrence was observed about 2 years before. Poor patient compliance can lead to complications and negative outcomes. Therefore, long-term follow-up is indispensable to early recognition of local recurrence. This case report aims to emphasize the importance of patient compliance after basal cell carcinoma excision in order to prevent further tumor growth.
