Submucosal neuroendocrine tumor (NET) is an uncommon cause of GI bleeding. These are small but significant lesions because metastasis can occur when the size is around 15mm. The location in the duodenum can pose diagnostic difficulties because it is in a "blind area" imposed by an acute turn in the "C loop" anatomy. A small submucosal "bump" can be missed in the duodenum or be mistaken for the papilla of Vater. Cap fitted gastroscopy, first described in 2000 by the author, is now a simple technique widely used to improve visualisation and targeting of lesions for biopsy and endoscopic therapy.
