This video describes the stage wise management of Endometrial Cancer which is a type of cancer that begins in the uterus. Stage 1 – tumor confined to uterus. 1a - less than 50% myometrial invasion TAH+BSO , 1b - more than 50% myometrial invasion TAH+BSO + AR Note: adjuvant radiotherapy (AR) is given in high risk patients (grade 3, deep myometrial invasion, cervical extension, serous or clear cell histology) Stage 2 – tumor invades cervical stroma – Radical Hysterectomy + BSO + Pelvic Lymphadenectomy + AR , Stage 3 – local / regional spread of tumor , 3a – invades uterine serosa and/or adnexa - Neoadjuvant Chemotherapy followed by radical hysterectomy + lymphadenectomy + infracolic omentectomy + AR , 3b – invades vagina and/ or parametrium - Chemoradiation
3c1 – positive pelvic nodes - Chemoradiation , 3c2 – positive para-aortic nodes with or without +ve pelvic nodes , Stage 4 – bladder and/or bowel mucosa and/or distant metastasis – Chemoradiation
4a – invasion of bladder and/ or bowel mucosa , 4b – intra-abdominal metastases and/ or inguinal nodes .
