This video is about the hurdles I overcame while finding a solid exercise routine for myself by going to a gym in my community, considering the challenges of wearing a prosthetic while exercising. I have gotten to where I can go to the gym comfortably now, and wanted to encourage you to do the same if you were thinking it was impossible! (Remember to consult with your doctor before doing any physical exercise after mastectomy!)

I've read a lot of books about planning for fitness and avoiding recurrence, and by far the best book I've found on the subject is by Dr. Keith Block, MD out of Chicago. His book, "Life Over Cancer" is excellent in many ways, but especially the detailed attention that he gives to the need for certain kinds of physical fitness. Maybe I'll do a book review video of this book some day soon!

Cáncer de testículo - Signos, Síntomas, Diagnóstico y Tratamiento - Cáncer testicular

Aproximadamente 74500 hombres son diagnosticados con cáncer de testículo cada año, mientras que 9300 hombres mueren a causa de dicho padecimiento.
Siendo la causa número 28 de nuevos casos y 34 de muerte entre todos los tipos de cáncer y el tumor sólido más común entre 15 y 35 años.
A travez de los años ha mejorado la sobrevida de los pacientes, 1970 era la causa 11 de muertes con sobrevida a 5 años del 65%, cambiando a una sobrevida de 95% a 5 años.

Y esque como pueden estar viendo en la imagen, los principales tipos de cáncer en hombres son Pulmón (14.3%), Próstata (14.1%), Colorectal (10.6%), Estómago (7.1%) e Hígado (6.3%).

Como les comentaba, puede que sea el número 28 de nuevos casos de cáncer, pero no por eso deja de ser importante ya que es uno de los tumores que más afecta a los hombres, muchas veces pasa desapercibido y en algunos casos se detecta de manera tardía.

En esta ocasión les hablaré un poco sobre el cáncer testicular, síntomas, signos y tratamiento.

95% del Cáncer a nivel testicular son tumores de células germinales. Estos se dividen en seminomas y no seminomatosos.

Hay otros tipos de tumores que incluyen Tumores de Leydig, gonadoblastoma tumores metastásicos, carcinoides y linfoma.

Epidemiología: Incremento progresivo del cáncer testicular en las últimas décadas. Los factores aún se desconocen pero hay una gran variedad de hipótesis propuestas (exposición intra uterina de Dietilestilbestrol, exposición temprana a virus ambientales o trauma testicular). Hasta este momento ninguno de dichos factores se ha logrado asociar de manera contundente.
Aparentemente la incidencia menor es en África y Ásia y mayor en Alemania, Suiza y Nueva Zelanda.

Factores de riesgo:
-Criptorquidia: Posición anormal del testículo. Cuando se identifica se recomienda una orquiectomía profiláctica, especialmente si el testículo está en el abdomen, a nivel inguinal puede optarse por vigilancia.
-Hipospadia: Alteración donde la uretra no está en la punta del pene.
-Cáncer contralateral.
-Historia familiar de cáncer testicular: Aproximadamente 1-3% de los pacientes, tiene un familiar directo con cáncer testicular.
-Padecer otro tipo de cáncer.
-Infección por VIH: Riesgo incrementado en personas que viven con VIH.
-Microlitiásis testicular: Litos de pequeño tamaño se han asociado con cáncer testicular, aunque se desconoce su relación directa ya que la mayoría de pacientes con síntomas testiculares acudirá a revisión y se puede encontrar microlitiasis.
-Enfermedades genéticas: Síndrome de Down, Síndrome de Klinefelter, Complejo de Carney son solo algunas de las enfermedades genéticas que se relacionan.
-Hipercolesterolemia.

Síntomas
Cualquier tumoración testicular debe considerarse como cáncer hasta demostrar la contrario. Los tumores testiculares se presentan como una masa dura, hinchazón del testículo que se puede notar accidentalmente. 30-40% de los pacientes tiene sensación de plenitud abdominal y 10% se presenta con dolor agudo.
10% de pacientes tiene datos de metástasis como: Tumoración en cuello, tos, náusea, vómito, dolor lumbar, dolor óseo, afección a sistema nervioso central.
Ginecomastia en 5% de pacientes.


Diagnóstico
Revisión clínica.
Descartar otras causas: Torsión testicular, Epididimitis, hidrocele, varicocele, hernia.
Para el diagnóstico adecuado a menudo se usan métodos de imagen como ultrasonido, tomografía, estudios por medicina nuclear o marcadores séricos como alfa feto proteína, subunidad B de la hormona gonadotropina coriónica humána o la lactato deshidrogenasa DHL.

El tratamiento dependerá del grado de tumor, invasión a otros sitios y de la condición general de la persona.

In some types of cancers, the word "metastatic" is associated with other words like "incurable" or "life-threatening." In the context of prostate cancer, however, metastatic disease is almost always more vulnerable to treatment than other types of metastatic cancer.

This is especially true if the metastatic prostate cancer is limited to the pelvic lymph nodes. In fact, for patients with spread confined to the pelvic lymph nodes, there may still be a chance to achieve a cure or a long-term remission, and new technologies are likely to improve these patients' odds even further in the coming years.

In this video, medical oncologist Mark Scholz, MD, discusses how the PSMA PET scan and advancements in treatment delivery are providing physicians with more accurate disease staging and thus more specific treatment selection, which is offering hope that men with these kinds of high-risk and relapse cases will have even better outcomes in the coming years.
