Mesothelioma Treatment for Navy Veterans.

A Navy veteran who is diagnosed with mesothelioma has another tough battle to fight. While there is no cure for mesothelioma, there are several treatment options that can reduce pain and extend a patient’s life expectancy. However, the possibilities become more limited as the disease progresses.

Navy veterans could be candidates for surgery. Surgery can be a curative or palliative treatment option for patients with mesothelioma. Doctors may suggest any of the following procedures:

Extrapleural pneumonectomy: Surgeons remove the affected lung, part of the pericardial sac, part of the diagphragm and the tissues lining the chest wall.
Pleurectomy decortication (P/D): The pleura surrounding the lungs and any visible tumors in the chest cavity are removed.
Cytoreduction or debulking surgery: Diseased tissue in the peritoneal and abdominal area is removed. In some cases, heated chemotherapy drugs are inserted into the abdomen as well.
Pericardiectomy: Doctors remove all or a portion of the pericardium around the heart.
Radical inguinal orchiectomy: The diseased testicle and the entire spermatic chord are removed.

Chemotherapy is another option. Doctors may prescribe a variety of drugs and chemicals to attack and kill mesothelioma cells. Chemotherapy may be used before surgery to shrink the tumor or post-operatively to kill any remaining cancer cells.

Radiation therapy uses high energy X-rays or particles to attack cancer cells. Ionizing radiation is most effective at shrinking or preventing growth of mesothelioma tumors. Radiation therapy may be used in combination with other therapies or as part of a palliative care plan for Navy veterans with mesothelioma.

Palliative care focuses on improving a patient’s quality of life and easing pain during any stage of mesothelioma. Doctors may perform procedures to remove fluid build-up to make breathing, eating and moving more comfortable.

Clinical trials are research studies that explore new methods for treating mesothelioma. Navy veterans should ask their doctors if they are eligible to participate in any current or upcoming trials.

Navy Asbestos Medical Surveillance Program.

The U.S. Navy operates a medical monitoring program for military servicemembers with a history of occupational exposure to asbestos. The goal of the Asbestos Medical Surveillance Program (AMSP) is to identify signs of mesothelioma and other asbestos-related diseases quickly so that Navy personnel can get immediate treatment.

The AMSP keeps records of some Navy servicemembers who were exposed to asbestos. Eligibility to enroll in the program depends on whether the sailor or civilian meets certain exposure requirements. Those who qualify receive regular medical exams and chest X-rays to look for any signs of pleural mesothelioma or other asbestos illnesses.

AMSP doctors also use spirometry to check how well the servicemember’s lungs are working. Spirometry is a test that measures how much air patients can inhale and exhale. It also assesses how quickly a patient can exhale. Physicians can also use spirometry to evaluate how well a patient is responding to treatments for chronic lung conditions like COPD.

Doctors can identify Navy servicemembers at the greatest risk of developing asbestos-related diseases by reviewing their answers to a questionnaire and conducting a physical exam. Specially trained physicians, called B readers, review the X-rays.

All of the testing is kept on file for future reference. That gives doctors the best chance of catching any new or worsening symptoms of mesothelioma or other conditions caused by asbestos exposure.

Although Navy servicemembers make up the majority of all AMSP enrollees, the program also extends to members of the U.S. Army, Air Force or Coast Guard.

Life Expectancy for Navy Veterans with Mesothelioma.

Early detection is key to extending the life expectancy for people with mesothelioma. Statistics show that patients generally live 12 to 21 months after diagnosis. Less than 10 percent of mesothelioma victims will survive for five years.

However, patients who are caught in the earliest stages of mesothelioma have the best prognoses. At Stage 1 or Stage 2, mesothelioma tumors are relatively localized and not spread throughout the body.

Mesothelioma is a type of cancer that develops in the lining that covers the outer surface of some of the body's organs. It's usually linked to asbestos exposure.

Mesothelioma mainly affects the lining of the lungs (pleural mesothelioma), although it can also affect the lining of the tummy (peritoneal mesothelioma), heart or testicles.

More than 2,600 people are diagnosed with the condition each year in the UK. Most cases are diagnosed in people aged 60 to 80, and men are affected more commonly than women.

Unfortunately, it's rarely possible to cure mesothelioma, although treatment can help control the symptoms.

Symptoms of mesothelioma.

The symptoms of mesothelioma tend to develop gradually over time. They typically don't appear until several decades after exposure to asbestos.

Symptoms of mesothelioma in the lining of the lungs include:

1) chest pain.
2) shortness of breath.
3) fatigue (extreme tiredness).
4) a high temperature (fever) and sweating, particularly at night.
5) a persistent cough.
6) loss of appetite and unexplained weight loss.
7) clubbed (swollen) fingertips.

Symptoms of mesothelioma in the lining of the tummy include:

1) tummy pain or swelling.
2) feeling or being sick.
3) loss of appetite and unexplained weight loss.
4) diarrhoea or constipation.

See your GP if you have any persistent or worrying symptoms. Tell them about any exposure to asbestos you may have had in the past.

What causes mesothelioma?

Mesothelioma is almost always caused by exposure to asbestos, a group of minerals made of microscopic fibres that used to be widely used in construction.

These tiny fibres can easily get in the lungs, where they get stuck, damaging the lungs over time.

It usually takes a while for this to cause any obvious problems, with mesothelioma typically developing more than 20 years after exposure to asbestos.

The use of asbestos was completely banned in 1999, so the risk of exposure is much lower nowadays. But materials containing asbestos are still found in many older buildings.

How mesothelioma is diagnosed?

If your GP suspects mesothelioma, they'll refer you to a hospital specialist for some tests.

A number of different tests may need to be carried out, including:

1) an X-ray of your chest or tummy.
2) a CT scan – a number of X-ray images are taken to create a detailed image of the inside of the body.
3) fluid drainage – if there's a build-up of fluid around the lungs or in the tummy, a sample may be removed using a needle inserted through the skin so the fluid can be analysed.
4) a thoracoscopy or laparoscopy – the inside of your chest or tummy is examined with a long, thin camera that's inserted through a small cut (incision) under sedation or anaesthetic; a sample of tissue (biopsy) may be removed so it can be analysed.

These tests can help diagnose mesothelioma and show how far it's spread.

Treatments for mesothelioma.

The best treatment for mesothelioma depends on several factors, including how far the cancer has spread and your general health.

As mesothelioma is often diagnosed at an advanced stage, treatment is usually focused on controlling the symptoms and prolonging life for as long as possible.

This is known as palliative or supportive care.

Possible treatments include:

1) chemotherapy – this is the main treatment for mesothelioma and involves using medicine to help shrink the cancer.
2) radiotherapy – this involves using high-energy radiation to kill cancer cells; it may be used to slow the cancer down and keep it under control.
3) surgery – an operation to remove the cancerous area can be done if mesothelioma is detected at a very early stage, although it's not clear whether surgery is helpful.

You'll also probably have treatment for your individual symptoms to help you feel as comfortable as possible.

For example, regularly draining fluid from your chest may help your breathing and strong painkillers may help relieve your pain.

Sometimes a procedure is carried out to stop the fluid coming back again by making the outside of the lungs stick to the inside of your chest (pleurodesis), or a tube is put in your chest to drain the fluid regularly at home.

Answer. The product you're asking about is a plant-based calcium supplement enhanced with vitamins and minerals. A study published in the Journal of the American College of Nutrition in 2016 found that some people using the product did see an increase in bone density and that the product wasn't associated with adverse health effects. We encourage you to discuss this option with your health care provider, and also to explore nonbisphosphonate treatments, which are delivered via injection or infusion.

Meanwhile, exercise is an important component of bone health, and we're glad to know you've made it part of your daily routine. In addition to aerobic activities, be sure to include resistance and weight-bearing exercises as well.

Question. A couple of years ago, I started taking melatonin before bedtime. After a few months, I started to feel lightheaded and unfocused during the daytime. I stopped taking it, and I started to feel better. Have you ever heard of this before?

Answer. Although melatonin tends to be a benign supplement, it's efficacy as a sleep aid can vary depending on the dose and the individual. So do side effects, which can include the fuzziness and lightheadedness you describe. People have also reported daytime sleepiness, short-term feelings of depression and gastric issues. We've had a number of letters about poor sleep (no surprise, considering the year we've all just had), so we'll address nonpharmacologic approaches to insomnia in an upcoming column.

Question. A reader had a question in response to a column about bronchiectasis, which is when inflammation and infection cause the bronchial tubes of the lungs to become thickened. "My wife was diagnosed with bronchiectasis in 2007 and had three or four flare-ups per year that were treated in various ways," he wrote. "Since being treated with a new medication, she's had almost no mucus and only one minor flare-up in six years. Can bronchiectasis go into remission?"

Answer. Yes, the good news is that bronchiectasis can go into remission, which is when the signs and symptoms of a disease are reduced or disappear. However, remission is not a cure. Your wife's bronchiectasis is under control due to her medications and treatment protocols, so it's important for her to continue to adhere to them.

The lawyers in this office get very involved with our clients. We spend a lot of time with them, and we've seen hundreds and hundreds of people go through the journey of the medical treatment for mesothelioma. We're fortunate to have one of the top cancer hospitals in the country here in our backyard, Roswell Park. But the journey is a little complicated because mesothelioma can affect three different organs in your body. The lung lining called the pleura, the heart lining called the pericardium, and the stomach lining called the peritoneal. And there's different treatments. There's chemotherapy, there's surgery, there's radiation, there's immunotherapy.

I think it's important when you represent someone with one of these terrible diseases of mesothelioma, to have an appreciation of what they're going through and to accommodate them because their priority, obviously, is their health, not the lawsuit. And so we've seen firsthand what the clients are going through, and at times it's heart wrenching, and other times it's heartwarming because there is good news for some of the treatments and the advancements with chemotherapy and immunotherapy. Our firm has been doing this for more than 25 years, and during that quarter of a century, we've seen immense improvement and advancement with treatment.

Our clients are living longer. I always say you got to fight for another Christmas, you got to fight for another Easter. So you can be with your kids. You can see that first communion, you can see that graduation. You can see that holiday. So the medical treatment to our clients is super important.

We always say that our clients live their life by the Grace of daily obligations, especially in the Buffalo area and Niagara Falls. So many of the providers were working what we call a second front. A common symptom of mesothelioma is represented by recurrent pleural effusions, which are routinely submitted for cytological examination (smears and/or cell blocks). Since on cytology it is impossible to assess invasion by neoplastic mesothelial cells into sub-pleural tissue or lung parenchyma, establishing a definitive diagnosis of MPM by cytological examination alone remains controversial, especially in the light of the medico-legal implications correlated with the diagnosis of MPM. However, in selected cases in which more invasive procedures are contraindicated, the cytological diagnosis of MPM, which relies on a different set of both cellular and architectural features and is supported by ancillary techniques, can be performed, although its sensitivity is low compared to histology. In fact, the published sensitivity of cytology for the diagnosis of mesothelioma ranges from 30% to 75%. Moreover, in the cases in which histology is not available, a close correlation with clinical and imaging findings is essential for a definitive diagnosis.

Not all MPMs exfoliate tumor cells in the pleural cavity; particularly, the malignant cells in sarcomatoid mesothelioma tend not to be shed into the effusion fluid. Thus, the effusion could only contain reactive epithelioid mesothelial cells, and these cells may mislead the pathologist. In these cases, a core biopsy (or larger specimens) is necessary to establish a definitive diagnosis, particularly when surgery is considered, because the presence of a sarcomatoid component may influence therapeutic management.

There are several cytological features in pleural effusions that raise varying levels of suspicion for MPM, such as the extent of mesothelial proliferation, the presence of papillary structures, scalloped borders of cell clumps, intercellular windows, variation of cytoplasmic staining and its density, and low nuclear-to-cytoplasmic ratios. However, some of the cytomorphological findings of MPM are shared between reactive and malignant epithelioid mesothelial cells. Therefore, the differential diagnosis of mesothelial proliferations may be very difficult or even impossible in cytological specimens, underscoring the importance of ancillary techniques to clarify diagnosis.

The application of immunocytochemistry (ICC) and molecular methods, such as fluorescent in situ hybridization (FISH) performed preferentially on cell blocks, increases the diagnostic accuracy of cytology. The differential diagnosis of MPM and the use of ICC and molecular markers in cytological samples are the same as in histological specimens. Several ICC markers, such as desmin, p53, epithelial membrane antigen (EMA), glucose transporter protein 1 (GLUT-1), insulin-like growth factor 2 messenger RNA-binding protein 3 (IMP-3), and CD146, have been proposed to assist in uncertain cases. Nevertheless, none of these markers, alone or in combination, appeared to be useful with sufficient confidence in the routine diagnosis of MPM.

Among new ancillary tests, FISH that shows the homozygous deletion of p16 (CDKN2A) and the loss of BRCA1-associated protein 1 (BAP1) expression by immunocytochemistry are particularly useful to differentiate mesothelial hyperplasia (MH) from MPM. These two markers were shown to be highly specific for MPM; however, their low sensitivity limits their clinical utility.

The cytological distinction between mesothelioma and secondary carcinoma is less problematic now than in earlier decades; overall, the sample is adequate for cell block preparation for various immunohistochemical studies. Because of the frequent litigation of cases of MPM and the availability of many mesothelial and adenocarcinoma markers, the guidelines strongly recommend that all cases should be confirmed with ICC/IHC.

MPM is a heterogeneous tumor, including three main histological subtypes: epithelioid (60–80%), sarcomatoid (10%), biphasic/mixed (10–15%), and other less common ones. The recognition of the various histopathologic patterns facilitates differential diagnosis and subsequent ancillary tests. The invasion of the chest wall soft tissue or underlying lung parenchyma is still the most reliable indicator of malignancy.

The relationship between mesothelioma and Simian virus 40 (SV40) is one that continues to be investigated. Some contend that SV40 infection can cause mesothelioma. Others have evidence that disputes this claim. The conflicting evidence suggests more research is needed.

Mesothelioma is a rare cancer that usually develops in the lining of the lungs and its primary cause is asbestos exposure. SV40 is a virus that contaminated polio vaccines between the 1950s and 1960s. Although the SV40-contaminated vaccines were successful in reducing the incidence of polio, questions regarding its safety began to emerge shortly after its use.

Background of Simian Virus 40.

Simian virus 40 is a virus that was initially discovered in 1960 in rhesus monkey kidney cells. At the time, Dr. Jonas Salk’s and Dr. Albert Sabin’s polio vaccines were made from these cells and the mass immunization program for polio had already begun. From 1955 to 1963, an estimated 10 to 30 million people in the U.S. were inadvertently exposed to SV40-contaminated polio vaccinations.

The exposure was so significant that researchers believe 80 to 90 percent of all U.S. children under the age of 20 had received at least one polio vaccination potentially contaminated with SV40 during those years.

When 1961 reports indicated the injection of SV40 into hamsters could cause tumors, the U.S. government instituted a screening program requiring all new polio vaccinations to be free of SV40. However, vaccinations made prior to this regulation were not withdrawn from the mass immunization program and children continued to be exposed until 1963.

Conflicting Evidence About SV40.

In the early 1960s, researchers found SV40 had the ability to transform hamster and human cells to induce tumor formation. After the discovery of SV40-like DNA sequences in mesothelioma samples in 1994, investigators have continued to test the causal relationship between SV40 and malignant pleural mesothelioma.

The amount of research that supports a causal relationship between mesothelioma and SV40 is significant enough to warrant concern. In animal testing, 100 percent of those injected with SV40 into the pleural space developed mesothelioma within three to six months. One study found that 60 percent of malignant mesothelioma cases in the U.S. contain SV40. While another study suggests 80 percent of cases contain the virus.

Tumor Studies Vary Greatly.

In some studies, researchers have found human mesothelial cells to be uniquely susceptible to SV40, and have suggested that the virus and asbestos may act as co-carcinogens. SV40 has been detected in human tumors in more than 40 different laboratories and many of the reports from these labs say they found SV40 in mesothelioma tumors. Although there is evidence of SV40 acting as a causative agent in the development of mesothelioma, there is an equal amount of evidence arguing against this causal relationship.

SV40 Working Group.

With the intent of resolving the conflicting information on mesothelioma and SV40, the International SV40 Working Group was formed in 1997. The group included the majority of laboratories studying SV40 in human tissues.

In a study funded by the National Cancer Institute, nine laboratories agreed to participate in the study, which tested tissue samples for traces of SV40. Each laboratory was given a set of 25 human mesothelioma samples and 25 normal lung tissue samples.

The samples were not labeled and each laboratory used their own method for detecting SV40. Results showed that none of the mesothelioma samples were consistently positive for SV40, suggesting a lack of evidence for a causal relationship between mesothelioma and SV40.

Vaccination Did Not Increase Cancer Incidence.

Other studies showed similar results. In one evaluation of children born during the years that SV40-contaminated vaccinations were used, an overall difference in cancer incidence was not reported when comparing the group to children who received uncontaminated vaccinations.

A 2011 study concluded exposure to SV40 is not enough to cause cancer on its own in humans, but that it might co-contribute to the development of cancer when a person is exposed to other carcinogens such as asbestos.
