This is an interesting case of acute appendicitis in an adolescent. There was a calcified fecalith identified at the base of the cecum and distally the appendix was dilated and edematous with scattered focuses of air in the appendiceal wall raising the possibility of pneumatosis and necrosis, also there was evidence of decreased contrast enhancement in some areas of the appendiceal wall. Patient was brough emergently to the ER where they found a contained rupture.
