Observation:
A well-defined, predominantly sellar mass is seen expanding the sella. The lesion appears hyperdense measuring~ 48 x 36 in max cross sectional diameter. There is evidence of non enhancing cystic areas seen right supero-lateral and infero-anterior aspect. Few calcific foci are seen in the lesion. It is eroding the sellar floor and dorsum sellae with extension into the posterior aspect of sphenoid sinus.

A pedunculated extension is also seen extending superiorly from the Right lateral aspect of the mass. It is extending into the cavernous sinus and the suprasellar cistern. There is indentation of the optic chiasm.
On post-contrast images, the mass and the extensions show moderate, heterogeneous enhancement. Right ICA is seen to encased by the lesion.


Interpretation: Findings suggestive of Sellar heterogeneously enhancing mass with cystic areas and extension as described likely of 

                                     Pituitary Macroadenoma
